To enroll in the program:

Step 1: The enrollment forms are completed via a state webpage process and are located at this website: http://www.nj.gov/dca/affiliates/uez/index.shtml

Step 3: Complete the page listed on the PDF and send to the UEZA via email.

Step 4: await UEZA approval via the state of NJ DCA, process approval takes about one month.

UEZ Business Recertification Process for Business in the Program

If your business is recertifying for yearly re-enrollment follow the steps listed below. This recert should be completed yearly and once enrolled your will be notify via mail on your yearly recert date per the State UEZA/ DCA offices.

Step 2: go to UEZ Program webpage:

Step 3: if you have any issues, please contact the UEZA Help Desk at 877-913-6837. Your application must be completed via the internet. No paper applications will be accepted.
